To delineate viral hepatitis co-infections among an immigrant cohort residing in southern Italy. All undocumented immigrants and low-income refugees requiring a clinical consultation at one of the five first-level clinical centers in southern Italy, consecutively evaluated from January 2012 to February 2020, were participants in a prospective, multi-center study. The study's cohort was screened for hepatitis B surface antigen (HBsAg), anti-hepatitis C virus (HCV) antibodies, and anti-HIV antibodies. HBsAg-positive individuals were also tested for anti-delta antibodies. Of the 2923 subjects who participated, a subgroup of 257 (8%) displayed only HBsAg positivity (Control group B), 85 (29%) presented exclusively with anti-HCV positivity (Control group C), 16 (5%) showed dual positivity for HBsAg and anti-HCV (Case group BC), and 8 (2%) exhibited a combination of HBsAg and anti-HDV positivity (Case group BD). In a related observation, 57 (19%) of the subjects were anti-HIV-positive.
